American Medical Association: Community Health Grants
The American Medical Association (AMA) Foundation aims to increase health equity and improve health outcomes among vulnerable populations utilizing evidence-based models of care. The Community Health Grants program will support innovative programs that target the prevention and reduction of type 2 diabetes and hypertension.
Amount: Grants of up to $60,000 will be awarded and are renewable for up to three years.
Eligibility: Community based organizations including free clinics that meet the following criteria:
- U.S. based nonprofit organization with 501(c)3 status that have been in operation for over three years
- Annual operating budget less than $3,000,000
- Provide services in the continental U.S.
