New Mexico Aging and Long Term Services Department: Alzheimer’s Disease and Dementia Services
The New Mexico Aging and Long Term Services Department is seeking to support an organization to provide an array of services and supports for persons with Alzheimer’s disease and other dementias, and their caregivers. Services and supports include information and assistance, support groups, care coordination, culturally and linguistically appropriate caregiver training, self-directed respite, and safety programs. Proposals must also address the provision of education and training regarding Alzheimer’s disease and dementias to Aging Network providers. All services and supports must be provided statewide by qualified and knowledgeable staff who have continuous training and support from a reputable source.
Amount: A total of approximately $622,759 is available to fund a single source contract award for services statewide for one year. Awards may be made for a term of one year with the option of seven additional one year extensions.
Eligibility: Public or private organizations are eligible to apply.
Note: A recommended pre-proposal conference will be held on February 25, 2016.
