Avon: Breast Health Outreach Program

The mission of the Avon Breast Health Outreach Program (BHOP) is to navigate medically underserved women and men to breast health education, screening, and treatment services. Medically underserved populations – including low income, un- or under-insured, and other marginalized populations – need targeted, customized approaches to enable them to obtain regular screening mammograms and clinical breast exams. Successful programs include:

  • Culturally-competent educational strategies
  • Dedicated outreach, education, and navigation staff with specialized language and communications skills who can reach women where they live, work, and worship
  • Ongoing relationships and tracking of clients over time
  • Personalized assistance to those who have difficulty accessing healthcare

The Avon BHOP seeks to support programs that:

  • Demonstrate the ability to overcome barriers to care for underserved women
  • Educate women about breast cancer, early detection and screening, risk-reduction, and healthy lifestyle choices
  • Provide outreach and patient navigation for underserved and hard-to-reach women to both first-time screening and annual screening
  • Develop partnerships between community-based outreach and patient navigation providers and local medical providers
  • Work with healthcare providers to ensure timely and appropriate clinical follow-up of abnormal screening results
  • Assist clients with referrals to other medical and social services as needed and when available
  • Educate older people about coverage of mammograms through Medicare, Medicaid, and other insurance programs and assist them in obtaining the service from providers who accept these forms of health insurance

Amount: Approximately 55 grants will be awarded that typically range from $30,000-$60,000 and average $45,000. The amount is commensurate with the number of individuals to be served (see the solicitation for more details).

Eligibility: Private, non-government, nonprofit organizations (with federal non-profit status) in the U.S., Guam, and Puerto Rico. At least 40% of grants will be awarded to organizations with operating budgets under $5,000,000. In addition, Avon BHOP seeks to fund projects in:

  • Cities that host an AVON 39 The Walk To End Breast Cancer event, including: New York, NY; Los Angeles, CA; Chicago, IL; Philadelphia, PA; Washington, DC; Miami-Ft. Lauderdale, FL; Atlanta, GA; Boston, MA; San Francisco Bay Area, CA; Cleveland, OH; Detroit, MI; Pittsburgh, PA; Arizona (statewide); and Texas (statewide)
  • States with high breast cancer incidence and/or morbidity (New Mexico is not among these states) that are found at the following links: http://www.cdc.gov/cancer/breast/statistics/state.htm and https://gis.cancer.gov/mapstory/breast/

Native American Tribes are encouraged to apply and may do so under their federal tribal status.
